Registration for Spring 2024 opens October 1, 2023!

Registration requires an active US Lacrosse Membership and you will need to know your child's membership number.  Membership sign up and renewal can be done HERE. Sign up using the same email you plan to register with Centerville Youth Lacrosse.  

Once you have your USA Lacrosse membership renewed for the season, log into this website (or create an account if you are new) in the top right corner and the 2023 Division should pop up. Complete the online registration process. 

New uniforms for both boys and girls for the 2023 season - these are included in your fees and we commit to using these jerseys for 2023 and 2024 seasons.

There has been a couple issues with the USA Lacrosse Number verification.  First, please ensure that the e-mail you registered on this site is listed as an e-mail under your child's USA Lacrosse membership. You can add an e-mail on usalacrosse.com by signing into your account, going to family management, choosing your child's account and then under "communication method" adding a new communication method (be sure to choose e-mail).

Following registration and before the first indoor practice, CVLAX must have the following 4 documents turned in for each player:

1) Physical Form 
2) Signed Lindsay's Law Form
3) Emergency Medical Authorization Form
4) Signed Ohio Department of Health Concussion Form
